Our doctor will try to call you twice at the number provided on the scheduling form. We will not issue refunds for missed calls or if you type your phone number incorrectly on scheduling form. We do not issue refunds for canceled appointments. Refunds will not be given for missed appointments not rescheduled 24 hours prior to the appointment time.  We suggest rescheduling a new appointment via our webpage

* Pain - Severe chronic or intractable pain of neuropathic origin or severe chronic or intractable pain

* Parkinson's disease

* Post-traumatic stress disorder

* Sickle cell anemia

* Terminal illness

* Tourette syndrome

New Patient: $125

Renewing Patient: $75

In addition to our doctor fees above, you must log into your patient profile on the PA DOH website and pay the $50 fee to the state.

We will send you a reminder email to schedule your renewal. You can begin the renewal process within 60 days of the expiration date on your current card. We recommend renewing your card at least 2 weeks in advance to ensure your new patient card is delivered before your current card expires. It can take up to 7-14 business days for your new card to arrive after the appointment with the doctor.

Schedule a Renewal appointment with our doctor. You do not have to renew with the doctor that originally certfied you in PA.
