Register with PA DOH
If you have not registered with the PA Department of Health (PA DOH) for a Patient ID number, please click on the weblink below and complete the registration prior to making an appointment with our doctor.

Schedule Appointment with our Doctor
Choose an appointment time that works for you. Fill out our quick and easy intake form and be sure to supply your Patient ID number, valid email address and phone number, which will be used for us to certify you with the PA Department of Health.
Answer the doctor’s call at your scheduled appointment time, which may occur up to 15 minutes earlier than your scheduled time. The doctor will call you from a phone number with a 717 area code, so please answer the call.

Complete Registration
After your certification approval with our doctor, you are required to pay a fee of $50 to the PA Department of Health (PA DOH) in order to process your card with the state. Pay the PA DOH here

Patients need certified yearly and must pay our doctor fees and the required PA State fees yearly.

Renewing Patient: 70
*After your appointment with the doctor, you must log into your patient profile on the PA DOH website and pay the $50 fee to the state. Pay the state here: PA DOH
Please see the last FAQ in this list regarding discounts available for PA State Fees. PA State fees are not discounts to our doctor's fees.

PA statute outlines all of the following conditions as applicable:
* Amyotrophic lateral sclerosis
* Anxiety disorders
* Autism
* Cancer (including remission therapy)
* Chronic Hepatitis C
* Crohn's disease
* Damage to the nervous tissue of the central nervous system (brain-spinal cord - with objective neurological indication of intractable spasticity and other associated neuropathies)
* Dyskinetic and spastic movement disorders
* Epilepsy
* Glaucoma
* HIV / AIDS
* Huntington's disease
* Inflammatory bowel disease
* Intractable seizures
* Multiple sclerosis
* Neurodegenerative diseases
* Neuropathies
* Opioid use disorder (for which conventional therapeutic interventions are contraindicated or ineffective or for which adjunctive therapy is indicated in combination with primary therapeutic interventions)
* Pain - Severe chronic or intractable pain of neuropathic origin or severe chronic or intractable pain
* Parkinson's disease
* Post-traumatic stress disorder
* Sickle cell anemia
* Terminal illness
* Tourette syndrome
* PA law also allows a physician to qualify a patient for a medical card if there are any other medical conditions “of the same kind of class.” That means conditions like anxiety and eating disorders can also qualify, depending on a physician’s judgment.

You will need a PA State Issued ID (such as a PA Driver’s License or PA ID Card) and a Patient ID number from the PA DOH.

Our doctor will try to call you twice at the number provided on the scheduling form. We will not issue refunds for missed calls or if you type your phone number incorrectly on scheduling form. Refunds will not be given for missed appointments not cancelled 24 hours prior to the appointment time. We suggest rescheduling a new appointment via our webpage

Refunds will not be given for denials by our providers for medical certifications

We will send you a reminder email to schedule your renewal. You can begin the renewal process within 60 days of the expiration date on your current card. We recommend renewing your card at least 2 weeks in advance to ensure your new patient card is delivered before your current card expires. It can take up to 7-14 business days for your new card to arrive after the appointment with the doctor.

Schedule a Renewal appointment with our doctor. You do not have to renew with the doctor that originally certfied you in PA.

Yes. Patients under the age of 18 with a serious medical condition may get medical marijuana through a caregiver. See CAREGIVER FAQs for more information.

No. Caregivers must be registered with the Pa Department of Health. Please see the Caregiver Information provided by the PA DOH at this link for additional requirements, including a criminal background check.

There is a PA State Program called PA MMAP and patients may qualify for a discount for the yearly required PA state fees. MMAP discounts may apply if you are enrolled in the following PA financial hardship programs:
Children's Health Insurance Program (CHIP)
Medicaid (PA Medical Assistance)
Pharmaceutical Assistance Contract for the Elderly/ Needs (PACE/PACENET)
Supplemental Nutrition Assistance Program (SNAP)
Women, Infants and Children (WIC)
Please note that the discounts DO NOT apply to our doctor fees.
